Master Client Agreement

Our Master Client Agreement must initially be completed and signed prior to receiving services from Second Home Pet Resort; it provides information on the terms and conditions applicable to all boarding, grooming and day care services and remains in effect for all current and future stays at the resort for all pets in your household.  We encourage you to read this agreement in its entirety before arriving at Second Home Pet Resort.  Please print out and bring a signed copy of the entire agreement with you when your pet checks in.

Pet Personality Profile

A Pet Personality Profile must be completed for each pet who will receive services at Second Home Pet Resort.  You only need to complete this form once, although we will ask you during subsequent visits if any information about your pet has changed since you initially completed the form.  In order to expedite your check-in time, please print out and bring a signed copy of this document with you for each pet at check-in time.
